Medical A/B Forms must be submitted annually, and are required to be on file for all Troop 2012 activities.

Medical A/B/C Forms are required for all camps lasting over 72 hours. Including but not limited to Summer Camp.

Permission Form must be submitted annually for Scouts to be able to participate in Troop 2012 activites.
